1. Aadhar Card: Child, Mother & Father
2. PAN Card: Mother & father
3. Other Child's Documents:
Last Year's Report Card, Birth Certificate, Caste Certificate, CBSE registration card.
4. Residential Address Proof such as Latest Electricity or Telephone Bill or Rent Agreement.  
You will be asked to make a payment at the end of this form.
** Admission subject to verification of documents, availability of seats and payment of registration charges. Confirmation shall be sent by the school if admission is granted. All refunds subject to school's refund policy.

I accept the fee structure payable by me to the school towards my child's new admission to the class mentioned in the admission form. Due to the COVID-19 Pandemic lockdown, I am unable to visit the school to complete my ward's admission formalities and more over, the school's are observing compulsory shutdown.

I promise to submit all the below mentioned self attested hard copies and/or other documents along with the fees towards the admission of my child without any delay once the lockdown is lifted by the Government.

In this situation I have requested the school to connect my child to the school's ongoing online lecture and study support program in the view of my child's academic welfare.  

I am aware of the school's rules, regulations and policies and promise to abide by them.

2. PAN Card: Mother + Father

3. Residential Address Proof

4. Child's Medical Fitness Certificate

5. Transfer Certificate from School

6. Child's Last Year's Marks Sheet

7. Transport Contractor Form
